# The critical importance of diagnosing hearing loss early.

Hearing is essential for communication, social connection, safety and overall quality of life. Yet hearing loss is often neglected, because in most cases it develops so slowly and gradually that the person adapts without noticing the deficit. On average, several years pass between the first signs and the first visit to a specialist – years during which the brain is deprived of full auditory stimulation.

      
### How does hearing work?

      
Sound waves travel through the ear canal and set the eardrum (tympanic membrane) and the small bones of the middle ear in motion. This mechanical signal is transmitted to the cochlea in the inner ear, where delicate sensory cells convert it into nerve impulses for the brain. We distinguish two main types of hearing loss: **conductive hearing loss**, in which the passage of sound through the outer or middle ear is obstructed (earwax, infections, fluid behind the eardrum), and **sensorineural hearing loss**, in which the sensory cells themselves or the auditory nerve are damaged. The first type can often be fully treated; the second requires diagnosis as early as possible in order to preserve what remains.

      
### The first signs of hearing loss

      
Do not wait until the hearing loss becomes severe. Early indicators that call for an evaluation by an ENT specialist include:

      

        - Constantly asking others to repeat themselves, especially in noisy settings such as restaurants or family gatherings.

        - Keeping the television or radio at a volume higher than other family members prefer.

        - Difficulty following conversations on the telephone.

        - A feeling of blockage in the ear, or ringing, whistling and buzzing sounds (tinnitus).

        - The effort of listening tires you out – by the end of the day you feel exhausted from conversation.

        - You begin to avoid phone calls, meetings and noisy environments.

      

      
### What are the most common causes?

      

        - **Earwax (cerumen):** The simplest cause, fully treatable within a single visit.

        - **Middle ear infections and fluid:** Particularly common in children, where they can easily go unnoticed.

        - **Noise exposure:** Working in loud environments and listening to music at high volume irreversibly damage the sensory cells.

        - **Presbycusis:** The natural decline of hearing with age, usually after the age of 60, which first affects high-pitched tones and the clarity of speech.

        - **Otosclerosis and other ear diseases:** Stiffening of the middle ear bones, often treatable with surgery.

        - **Ototoxic medications:** Certain antibiotics and other therapies can damage hearing and therefore require monitoring.

      

      
### The link between hearing and cognitive health

      
Major international studies of the past decade have ranked untreated hearing loss among the leading modifiable risk factors for cognitive decline and dementia, including Alzheimer's disease. The mechanism is multifactorial: a brain deprived of auditory stimulation works harder to decode words at the expense of memory, while the social isolation that follows communication difficulties markedly reduces mental activity. Treating hearing loss in time is not merely a matter of comfort – it is a direct investment in the long-term health of your brain.

      
### Why is early diagnosis critical in children?

      
In children, even a mild or one-sided hearing loss directly affects speech development, learning and behaviour. Silent fluid behind the eardrum (otitis media with effusion) is the most frequent cause, and often the only signals are falling school grades or poor concentration. Any suspicion raised by parents or teachers deserves an audiological check-up – it is quick, painless and possible at any age.

      
### How is it diagnosed and treated?

        - **Microscopic otoscopy:** A detailed examination of the ear canal and eardrum to rule out simple causes such as earwax or infections.

        - **Pure-tone and speech audiometry:** Painless tests that precisely determine the degree and type of hearing loss.

Depending on the cause, treatment may be simple (earwax removal), medical (for infections and middle ear fluid), surgical (repair of the eardrum or the ossicles), or may involve modern hearing aids – devices that are now almost invisible and digitally fine-tuned to match your audiogram exactly. ### How can you protect your hearing?

      

        - Follow the 60/60 rule with headphones: no more than 60 percent of maximum volume, for no longer than 60 minutes at a time.

        - Use hearing protection at noisy workplaces and at concerts.

        - Do not use cotton buds inside the ear canal – they push wax deeper and endanger the eardrum.

        - Treat ear infections promptly and have your hearing checked periodically after the age of 50.
